To all members:
In preparation for the integration into Myna health insurance cards from December 2, 2024, we will ask members to check that their Individual Numbers registered with the Suntory Health Insurance Association as of October 1 are correct.
As described below, Notices of Eligibility Information and Requests to Check Individual Numbers (My Numbers) will be sent to employees who have registered their Individual Numbers (including numbers of dependent family members) with the Suntory Health Insurance Association. Please check the Individual Numbers (last four digits) shown and notify the Suntory Health Insurance Association if any number is incorrect.

1. Date to be sent: Late October to early November 2024
2. Document to be sent: Notice of Eligibility Information and Request to Check Individual Numbers (My Numbers)
3. Requests: Please check the Individual Numbers (last four digits) for yourself and for your dependent family members shown in the document. Notify the Health Insurance Association if any number is incorrect.
4. Information shown:
A. Member health insurance eligibility information (such as code, number, and branch number)
B. Individual Numbers (last four digits) registered with the Health Insurance Association

5. Other matters
・ These documents will be delivered to the address specified by each company. Please receive them through your company.
(For Voluntarily and Continuously Insured Persons, they will be sent by post to the addresses registered with the Health Insurance Association.)
・ These documents will be enclosed in a single envelope for up to four persons per household.
(They will be sent in multiple envelopes if your household, including dependents, is composed of five or more persons.)
・ These documents will not be sent to members whose Individual Numbers are not registered with the Suntory Health Insurance Association.
The Health Insurance Association is notified of its members’ Individual Numbers by their companies. If you have not yet submitted your Individual Number to your company, please do so as soon as possible.
