Family membership

Health insurance provides insurance benefits not just to insured persons, but to their dependent family members. These family members are referred to as “dependents”. Family members must meet certain conditions related to the extent of “family relationship” and “income” before they are authorized as dependents.

Applying to certify a spouse as a dependent

Required documents:
Documents to attach for dependent certification(see “Documents required when applying for dependent certification”)
Deadline:
As soon as possible after the reason for adding the family member arises
  1. Note: If the Health Insurance Association receives the documents and certifies the dependent within one month after the reason for adding the family member arises (including the date the reason arose), certification will be retroactive to the date on which the reason arose.
    However, if the Association receives the documents and certifies the dependent after more than one month after the reason for adding the family member arises, the date of certification will be the date on which the Association received the documents and certified the dependent.
Applies to:
Insured persons naming spouses as dependent family members under health insurance due to marriage, the spouse leaving employment, etc.

Applying to certify a child as a dependent

Required documents:
  • Application form (Form 2) (Not required when applying for a newborn child)

As soon as possible after the reason for adding the family member arises
  1. Note: If the Health Insurance Association receives the documents and certifies the dependent within one month after the reason for adding the family member arises (including the date the reason arose), certification will be retroactive to the date on which the reason arose. However, if the Association receives the documents and certifies the dependent after more than one month after the reason for adding the family member arises, the date of certification will be the date on which the Association received the documents and certified the dependent. (However, the date of certification for newborns is the child’s date of birth.)

Adding a family member other than a spouse or child

Required documents:
Documents to attach for dependent certification(see “Documents required when applying for dependent certification”)
Deadline:
As soon as possible after the reason for adding the family member arises
  1. Note: If the Health Insurance Association receives the documents and certifies the dependent within one month after the reason for adding the family member arises (including the date the reason arose), certification will be retroactive to the date on which the reason arose. However, if the Association receives the documents and certifies the dependent after more than one month after the reason for adding the family member arises, the date of certification will be the date on which the Association received the documents and certified the dependent.
Applies to:
Insured persons naming family members within three degrees of consanguinity as dependents
