Removing a family member as a dependent

You must complete the necessary procedures to remove a family member as a dependent if he or she no longer is or needs to be a dependent or no longer eligible as a dependent for reasons such as employment, death, or earning income above the income levels for dependent certification.
Additionally, please take the steps necessary to remove a family member receiving compensation for loss of income such as employment insurance (unemployment benefits), Maternity Allowance, or Injury and Sickness Allowance from your list of dependents, depending on the amount of compensation he or she receives (see the dependent certification criteria).

Log in to MY HEALTH WEB <JAPANESE> and apply using the Notification of Health Insurance Dependent (Change) (Removal) from the Notification of Health Insurance Dependent (Change) (Addition/Removal).

Required documents:
Health insurance card (for the dependent to be removed)
Elderly Benefits Card, Certificate of Application of Maximum Copayment Amount, Certificates Issued for Specific Disease Treatment (if issued)
Notes:
Attach the following documents depending on the reason for removal:
  • Beginning employment: health insurance card issued by employer (copy)
  • Income exceeding the standard amount: documentation of date on which income began to exceed the standard amount
  • Start of receiving unemployment benefits: both sides of certificate of eligibility for employment insurance benefits (copy)
  • Divorce: official documentation of the date of divorce (e.g., certificate of acceptance of divorce notification)
  • Death: documentation of the date of death
    *You may be asked to submit other documents that can be used to confirm the date of dependent removal.
Deadline:
As soon as the reason for removing the family member arises
Applies to:
Insured persons with dependents who meet the following descriptions:
  • No longer eligible as dependents due to reasons such as employment, living apart from the insured person, death, or other reasons
  • No longer satisfy the conditions for certification as dependents due to increased income
  • No longer rely on the income of the insured person for their livelihood because the insured person no longer provides an allowance
  • Start of receiving unemployment benefits
