Required documents:	Claim for Injury and Sickness Allowance/Additional Sum Injury and Sickness Allowance Claim Status Report ** The application forms will be provided by your employer’s HR section when your absence begins.
Deadline:	Have the forms certified by a doctor each month (for the period from the first through the last day of the month) and submit as instructed.
