If you lose your health insurance card
Important notice: Starting April 1, 2017, you will be charged a fee if you need to have your health insurance card reissued.
Immediately take the procedures described below if you lose your health insurance card or if your card is damaged/worn.

If you lose or damage your health insurance card or if you are unable to return it
Complete the following procedures if you lose your health insurance card (complete only steps ② and ③ if your card is damaged):
- ① Notify the nearest police station (optional).
- ② Pay the card reissue fee (1,000 yen/card; remittance fees must be borne by the insured person) into the designated bank account.
- ③ Submit the Application Form for Reissue of Health Insurance Card to the Association. (Attach documentation certifying remittance of the fee described under ② above; photocopies accepted.)
Note: Describe the circumstances under which you lost or damaged the card in as much detail as possible.
Your card will be promptly reissued after you complete these procedures and the Association receives the necessary documents.
If you find the lost health insurance card later, immediately return the found card to the Association. Note that the reissue fee will not be refunded. If you are no longer an insured person because you retired or for other reasons and you have lost your health insurance card and are unable to return it to the Association, fill out the required information on the following documents and submit to the Association.

- If your card is worn, it will be reissued promptly after you submit the application.

If you address has changed | Change the address by yourself in the address space on your card. Notify the Health Insurance Association if there is no room left to change your address. You will be sent an address sticker to affix to the card. Current employees do not need to notify the Association of address changes. Voluntarily and Continuously Insured Persons are requested to notify the Association of address changes. |
